In December 2019, a viral infection spread in China named novel coronavirus (COVID-19) or the severe acute respiratory syndrome coronavirus 2 (SARS-COV-2). This infection rapidly spread throughout the world and become COVID pandemic. The first case in the United States was noticed in January 2020 and presently (May 2021) has resulted in 32,933,337 cases and 586,793 deaths. (www.cdc.gov). Currently, India is facing similar situation how it was in the United States in 2020 and some lessons can be learned from the USA that can help Indian dentists too.

The effect of pandemic has also seen in dental practices across the nation. At the beginning of the first wave of COVID-19, approximately 198,000 dentists and dental specialists in the USA have to shut down their practices. The reason for the closure was the nature of the work performed in the dental care settings because dentistry is the field where physicians or other staff members have to work closely with the patient especially with the close proximity to the oropharynx and nasopharynx. The virus that causes COVID-19, SARS-CoV-2 is present in the nasopharynx and salivary secretions of the oropharynx. The unique nature of dental interventions involves the usage of various equipment that are known for aerosol production, such as high-speed handpieces, bone drilling equipment, and cavitron. These aerosolized SARS-CoV-2 particles can stay in the air for several hours and can cause the transmission of infection from an infected person to a normal person. Such a work environment is a high risk for the dental personnel teams and for patients for being infected with COVID.

New face of Challenges for dental practices

The pandemic changed the challenges for dental practices such as obtaining dental supplies, maintaining patient’s routine dental visits, financial sustainability, retention of employees, and patient safety challenges. Usually in dental practices, standard universal precautions applied to minimize the spread of infection but to minimize the COVID-19 infection, new strategies need to be implied. This is because the majority of COVID infected patients remain asymptomatic due to the long incubation time period of the virus (2-14 days) and can spread the infection without being aware of it. That’s why regular and daily screening of dental staff members and patients is necessary to ensure that they are neither asymptomatic nor being in contact with COVID patient or have travel history in the past 14 days.

Need of new strategies to spread awareness

 Various strategies have been implemented by dentists to continue attending to their patients and keep their businesses open. Various guidelines have been issued by various healthcare associations, to help dentists to practice dentistry and addressed the needs of their patients while minimizing the risk of transmission of disease. However, there is still a need to spread awareness, especially for the dentists or dental healthcare workers who are working in remote areas, about how to update their work field to continue providing services to their patients or community. The ensuing recommendations can be followed:

  • Triage patients before treatment, record temperature or any other COVID associated symptom, patient history of any contact with COVID patient or traveled outside the USA in last 15 days.
  • The use of double barriers for intra-oral X-ray sensors are recommended or if possible, perform extra-oral radiographs are recommended.
  • Wear PPE kits and change them regularly after every patient.
  • Hand hygiene that includes the washing of hands with soap for 20-30 seconds and use of hand sanitizer with the strength of 70% alcohol is recommended.
  • The use of pre-operative mouth rinse of patients by either using 0.5-1% hydrogen peroxide or 0.2% povidone iodine solution, to reduce the risk of spread of microorganisms during dental treatment.
  • Avoid aerosol-producing equipments and use of hand equipment is encouraged. If possible, try to schedule an appointment for aerosol-producing treatments at the end of the day.
  • Medical grade high efficiency particulate air (HEPA) filtration system along with high emergency ultraviolet light units or panels can also be installed. This system is known for 99.995% filtration rate and units can kill live bacteria or aerosolized bacteria trapped in the filter.
  • Negative pressure rooms or airborne infection isolation rooms can limit the spread of airborne infection to one room. These rooms can suck out 30-40% of air and ensure a one-way flow of air.
  • Include tele-dentistry to offer various services such as to provide urgent care services or follow-up services.

Lessons Learned from pandemic

Various lessons can be learned from this pandemic, that can help the dental care professionals to be prepared for any future crisis such as, analyze the practice operations which will help improve the financial and clinical performance. By improving the quality of work, focusing on reporting, performance indicators and benchmarking, cost-effective strategies can be implemented. By removing the financial barriers and improving the access to care can also help both patients as well as dentists. By providing phase treatment and breaking up the expensive treatment plans into series of smaller installments, incorporating technology such as tele-dental visits, offering competitive fees, affordable payment plans or collaborating with insurance companies can help solve these issues.  As of now, with every passing day, more people are getting vaccinated. It is expected that once we will be able to fight this pandemic, likewise other business, dental practices will also be able to open and see their patients as they do before COVID.

Students Corner: Process For Getting Migration Certificate From MUHS Nashik



A Migration Certificate is required in case you are switching from your previous university to a different university for further education or any other reason. It's a kind of "No Objection Certificate" that has to submitted for joining new university. In this blogpost I am going to share the process on how to issue Migration Certificate from Maharashtra University Of Health Sciences, Nashik. But don't worry, even if you belong to any other university, the overall process is very much common. Here are the steps that you have to follow:

STEP 1: You can download the application from official MUHS website or Click Here To Download. Once you have the hard copy of the application form, fill in the details and follow step two

 

STEP 2: You will have to issue a No Objection Certificate/Letter from your college authorities by filling an application for “Issuance of Migration Certificate”.  Make sure all the details are filled correctly with signature of Dean with College stamp. 


STEP 3: Once you have the application form and NOC ready as mentioned in step1 & 2 respectively; attach the bellow mentioned documents.

 

1. Original College Leaving Certificate Specially mentioned with “ISSUED ONLY FOR MIGRATION PURPOSE”

2. Attested copy of Statement of Marks of the Last Examination appeared.

3. Attested copy of the University Internship Completion Certificate in case passed the Final Year Examination.

4. Original Affidavit by a Bonded candidate of Govt./ Corp. College, if any. 

5. Letter/ Undertaking of Applicant’s College Authorities in Original.

6. Original “Affidavit Gap Certificate”, if any.

7. In case Duplicate Transfer/Leaving Certificate is submitted, attach the original F. I. R copy, original affidavit towards Duplicate Transfer/Leaving Certificate.

 

STEP 4:  Once you are done with above steps, you will have to submit the application form with all the mandatory documents along with a Demand Draft (Demand Draft should be from any Nationalized Bank drawn in favor of “The Registrar, Maharashtra University of Health Sciences” payable at Nashik.) on below mentioned address.

 

DEMAND DRAFT AMOUNT:

1)      By registered / Speed-post =  1000 INR

2)      Urgent / Speed-Post =  1500 INR

I have come across many incidences where professionals had to pay additional interest to the government as they failed to pay their professional tax and this happens because most of them were unaware of Professional Tax. Some also have misconception that taxes are to be paid only when their income comes in the tax bracket & they approach CA only when their income becomes taxable under Income tax. This blog is an attempt to expound the concept of professional tax and help you save on penalties.

What is Professional Tax Or PT?

PT is a tax charged on the income earned by way of profession or employment. This is additional to income tax and has to be paid to State Government. Professionals like doctors, lawyers, chartered accountants, artists etc comes under the purview of this tax. This is a slab-based tax and the rates vary state to state however the maximum amount that can be charged as PT by any state is Rs. 2500 annually. 

 

Registration for Professional Tax 

It is advisable to register your practice for PT immediately after commencement of dental practice. Before registering yourself for PT, you need to understand two main things one is PTRC and the other is PTEC. 

Professional Tax Registration Certificate (PTRC) is to be obtained by the professional in the capacity of employer. For instance, if you have 10 employees working with you in your business having monthly salary more than 7500, then you need to deduct PT on their salaries every month as per the rate given (Rs.200 per month & Rs.300 for February month).

Professional Tax Enrollment Certificate (PTEC) is to be obtained by the professional for its own existence and engaging in profession (business) within the state of Maharashtra.

PTR will have to be paid according to the salary paid to employees whereas PTE is fixed at Rs.2500 annually.

 

How to register? 

Step 1: Log on to www.mahavat.gov.in and open ‘e-PT Registration’ in e-services and fill in details like Name of the applicant, PAN, TAN, Status of Applicant and Location. Select Act as ‘PTRC/PTEC Registration’ (any one which you have to register for), the e-application for registration in ‘Form I’ will open for PTRC and ‘Form II’ for PTEC. Fill in the complete details and ‘Submit’.

 

Step 2: Once the application is submitted; an acknowledgement is generated containing date and time for attending before registration authority along with designation and address of registration authority. This acknowledgement has to be printed and signed by the applicant and produced before the PT registration authority at the time of verification on given date and time along with relevant documents. Once it is approved by the authority, TIN (Registration Certificate Number under the profession Tax Act) is generated. 

 

Step 3: After obtaining Registration Certificate the certificate holders are now needed to enroll themselves for the e-services on the same website, this will enable them to file e-return and e-payment of professional tax. 

 

The due date for paying PT is before 30th June of every financial year.

 

Consequences of late payment or non-payment of PT:

For late payment of PT, interest @1.25%-2% pm is levied and the state may also impose a penalty of 10% of the total amount due.

Moving abroad has its advantages and disadvantages too..

The primary considerations, that everyone does are the financial calculations, eligibility criteria, entrance exams and Visa for the respective country. If you guys are really interested in going abroad, I would recommend, please kindly do your research properly. Keep these things in mind while doing your research.

 

1. First, be sure which course you want to do and from which country.

2. Then do research regarding which universities are good for those courses.

3. Join the communities of the university you are applying for, on Facebook or any other social websites where you can connect with the students who are already studying there and clear your doubts

4. There are lot of options in masters in health regarding courses. So be sure of the course which you are opting for, has good scope in that country and as well as in India. (If you are planning to come back after studies)

 

 

Are above considerations enough or something more has to be thought of?

If you ask me, I would say the earlier considerations are basic which will be there before joining any University abroad. But, the real things will come once you join your MS. I stayed in Melbourne for 3 years and my journey was amazing. It was full of ups and downs. It taught me about being independent, being fearless, to forget and to forgive and many things about love and life but the most important thing it taught me about is, with discomfort comes growth. With every challenging situation, comes a great lesson. It helps you to evolve as a person. To believe in yourself and start everything from scratch in a new country, on your own, is difficult. But the moment you get succeed, that powerful feeling has no comparison in the world. People will always tell you how it has changed their life for good but no one talks about the pain they have gone through. 

 

No matter how supportive your parents are, leaving far away from them and the fact that they are getting fragile and old will always make you feel guilty. At times, you might feel lonely and depressed, no matter how many friends you have and how often you socialize. There are times when no one is around and you need to deal with it on your own. So if you are thinking of pursuing Post graduation or MS from abroad after BDS; think about all the factors and make a wise decision.
